If you're a new client, please complete the following forms and bring them to your first therapy session.

PLEASE NOTE:  To prevent a delay in receiving services and/or incurring an non-clinical out-of-pocket expense, please have the New Client Packet completed and returned prior to the 1st appointment.  If the New Client Packet is not completed and returned prior to the 1st appointment, then the 1st appointment will be used to complete these forms with the provider.  This will result in client being charged an out-of-pocket fee of $40.00 because it is a non-billable service and is not covered by insurance.
